1. You are greeted by a friendly interviewer who will document the situation with you in detail.You don’t have to give your name and your call is not recorded.
2. The interviewer assigns you a reference number and asks your to make one scheduled follow up call.
3. The information is relayed to the Office of Institutional Compliance and Risk Services at UTSA where it is evaluated to determine the most appropriate means of handling the issue.
4. Using the reference number and scheduled call back date given to you by the interviewer, you will call for a follow-up.You may be asked additional questions or asked for additional information at this time.

1. You will be guided through the web reporting system step by step.
2. The web reporting system will ask a series of questions and you will fill in the answers.
3. You will be prompted to select which incident best describes the issue you are reporting (ie., Discrimination, Fraud, Policy Issues and others).
4. You have the option to remain anonymous or provide your name and contact information.
5. The information is relayed to the Office of Institutional Compliance and Risk Services at UTSA where it is evaluated to determine the most appropriate means of handling the issue
6. Please note, you will receive a reference number when you submit your report online.
7. Click to Report via the internet.
