with you in detail. You
don’t have to give your name and your call is not recorded.
- The
interviewer assigns you a reference number and asks you to make one
call back.
- The
information is relayed to the Office of Institutional Compliance and Risk Services at
UTSA where it is evaluated to determine the most appropriate means of handling of the issue.
- Using
the reference number and scheduled call back date given to you by the
interviewer, you call for a follow-up. You may be asked additional questions or asked for additional information at this time.

- The web reporting system will ask a series of questions and you fill in the answers.
- You will be prompted to select which incident best describes the issue you are reporting (ie. Discrimination, Fraud, Policy Issues and others).
- You have the option to remain anonymous or provide your name and contact information.
- The
information is relayed to the Office of Institutional Compliance and Risk Services at
UTSA where it is evaluated to determine the most appropriate means of handling of the issue.
