  • 05/07/2012: Added form to website (04/13/11 revision date) to allow prior months reconciliations to be completed. Form will be deleted on 07/01/12.

  • 05/01/2012: Deleted the form as it is no longer being used. See FMOG - Statement of Accounts (SOA) Reconciliation Process for more information.

  • 04/13/2011: Removed the print range settings.

  • 03/01/2011: Redesigned form to allow data to be copied from the SOA - Reconciliation Report (optional business process). Also, added a certification statement and signature field for the Account Administrator to sign monthly during the SOA reconciliation process (required business process).
