Annual Enrollment for Plan Year 2014-2015

Ended at midnight on July 31, 2014

 

Although Annual Enrollment is officially over, you still have the ability to view your current and future benefits through MyUTBenefits. Please note important dates below:

  • Review confirmation statement
  • The deadline to submit Proof of Dependent Relationship documents through MyUTBenefits is August 15, 2014.
  • The deadline to submit electronic Evidence of Insurability through MyUTBenefitsis August 15, 2014.
  • The deadline to submit your Long Term Care Application directly to CNA is August 15, 2014.
  • Any new elections or rates are effective September 1, 2014 and deductions will be reflected in your October paycheck.
  • Expect ID cards for new plans elected & UT Select BCBS no later than September 1, 2014
    • Keep your ID cards for the plans you did not change
    • Keep your UT FLEX debit card if you are continuing enrollment and the card has not expired

HIGHLIGHTS

  • UT Select Medical
    • Monthly premiums increasing.
    • NEW $100 co-pay for MRI & CT scans. Co-pay waived, if you contact a BCBS Benefits Value Advisor (BVA).
  • UT Select Delta Dental
    • Monthly premiums increasing.
    • No plan design changes
  • UT Select Delta Dental Plus plan
    • Monthly premiums increasing.
    • No plan design changes.
  • Delta Dental HMO
    • Monthly premiums increasing.
    • No plan design changes.
  • Short Term Disability
    • No monthly premium or plan design changes.
    • Evidence of Insurability (EOI) required for new enrollees.
  • Long Term Disability, Term Life and AD&D Insurance
    • No monthly premium or plan design changes.
    • Evidence of Insurability (EOI) required for new enrollees except AD&D.
  • Long Term Care
    • No monthly premium or plan design changes.
    • Evidence of Insurability (EOI) required for new enrollees.
  • UT Flex Health Care and/or UT Flex Dependent Day Care.
    • Your UT FLEX election(s) will not automatically carry forward!
    • Maximum for UT Flex Health Care is $2,500 and $5,000 for UT Flex Dependent Day Care.
    • Grace Period remains the same.
    • Debit Card provided to all participants at no charge for UT Flex Health Care.
    • No change to administration fee ($12 annually per account).

Note: If you add your spouse or dependent child to any insurance plans, you will be required to provide copies of relevant documents such as: marriage certificate, birth certificate, adoption paperwork or other legal documents.
