Occupational Health Program Risk Assessment Form

The University of Texas at San Antonio Occupational Health Program is designed to provide oversight for university personnel who are exposed to animals and certain biological, chemical, and physical hazards. All employees engaged in the aforementioned activities must complete this form. Employees may choose to opt out of participation in the program, but as a consequence, will be denied access to certain high-risk facilities or job activities.
